CUDA support is only available in the beta currently. Your driver is really old. Install the latest version.

if this happens something with your opencl drivers is wrong. In the past people had problems with a rouge intel opencl runtime.

don't report problems when using --self-test-disable. your driver is broken.

A failing self-check usually means the driver has a bug. Don't expect anything when disabling that. From that I gather, however, that you are still able to start hashcat and the error is suddenly gone...
