I think I see what the error is you did. You prepended the hash to the ciphertext, but you need to append it |

one is that pass + salt can not be longer than 55
but there is another one both pass or salt can not be longer than 32 |

you're wrong. i guess you want to crack ipb2 or mybb which are the most popular software to use md5( md5($salt) . md5($pass) );
